this Department via facsimile on July 1, 1996. You must notify this office at
least seven days prior to treatment if there are any changes/additions to the
list of cooperators;

     2. It is your responsibility to notify the appropriate regional office of
the Department at least 48 hours prior to any treatment. Enclosed is a list of
regional office contacts and phone numbers;

     3. The experimental permit expires December 31, 1997.

     If you have any questions regarding this experimental permit, contact
Michael Radzevich, of my staff, at (518) 457-7446.
